Me: 'Well, why don't you have a look? It's not difficult to find - YOU'RE STANDING RIGHT IN FRONT OF IT...?!??'

A wicked habit we have in #patientsafety is giving healthcare professionals a destination without providing a workable map to get there.

We really need to partner with human factors specialists & create these maps together.
