We can dance with the system, try to influence it in the ways we want, yet culture is an emergent property of it which we can never properly predict . The system continually surprises us with unintended consequences. Interventions help steer but of course they promise nothing.

And there is the reason, we all need access to HF practitioners. Safer practice, savings, better well-being for staff, increased retention, the list goes on.

I've worked with many amazing people who came into patient safety because of traumatic events. Some of them I now consider friends.

On the other, it’s simplistic. One of many reasons why effective HF benefits from appropriate expertise, and can’t be reduced to simple rules. Just like medicine.
