We can dance with the system, try to influence it in the ways we want, yet culture is an emergent property of it which we can never properly predict . The system continually surprises us with unintended consequences. Interventions help steer but of course they promise nothing.

On the other, it’s simplistic. One of many reasons why effective HF benefits from appropriate expertise, and can’t be reduced to simple rules. Just like medicine.

There was a paper in healthcare on ‘use errors’ where it was adapted slightly, but often used out of *that* context (use errors). FWIW, a few thoughts on each. 1/
