For (Hemodialysis access-induced distal ischemia) pts with arteriosclerosis in the forearm arteries, never try to perform ulnar/radial angioplasty. I always use PAI procedure with 4mm end of ePTFE at proximal axillary. Much more reliable than Miller operation.
