There are distinct phenotypes of polycystic ovarian syndrome (PCOS).

Assessing ovarian morphology is not needed to manage hyperadrogenic symptoms. So we don't need to order an ovarian ultrasound to best treat hirsutism.

One main reason is to exclude other conditions. The prevalence of these conditions is rather low, but we should still always think about them.

How to work-up androgen excess in a woman 🤔

Signs: acne, hirsutism, male-pattern baldness

Labs: Testosterone, DHEAS, 17-OHP, prolactin +/- screen for acromegaly (IGF-1), Cushing's (salivary cortisol)

What can we expect in LDL lowering with raising statin dose?

Not much effect. Increasing rosuvastatin 40➡️80mg will provide minimal LDL reduction.

The reference range for total testosterone in men is based on healthy men age 19-39 with normal BMI.

This range is a one-size-fits-all not broken down by age or BMI.

These numbers just statistics and not based on any clinical endpoints.

DDT was banned in the 1970s but it's still found in human fat tissue after 2 generations!

DDT, used for malaria control in the 1960s, is an obesogen.

DDT exposure to a grandmother is associated with increased obesity in the granddaughter. That's wild.
