If doctors can be sanctioned for a PAs clinical work, i.e need to review the patient again to avoid legal or statuatory punishment, then should I ever supervise a PA?

The MDU whats our legal position?
The BMA whats our union position?
GMC what is regulatory position?

MD. We’ve never had it so bad. 🇬🇧🇪🇺🇺🇦(@surfdoctor) 's Twitter Profile Photo

Emily Moseley Indeed. The main question the RCP and GMC have yet to answer is ‘what is the point of a PA?’ If everything they do has to be duplicated then they are a waste of billions of NHS money. If they were simply used as physician ASSISTANTS as intended, then there might be an argument.

So, and I mean this respectfully, what in gods name is the point of a PA? Who is this role trying to help? Because it’s certainly not me, and struggling to see how it’s patients either.

Deeply concerning that mass expansion of MAPs has been planned without corresponding & clear medicolegal guidance on them

It’s a poor reflection of how unclear current guidance is, that current debate is focussed around one case from 7 years ago

Much better guidance is needed

The GMC released the following document detailing their response to the MAP consultation

They argue that for any group to be considered for statutory regulation, 10 criteria should be met. Let's look back and see which PAs meet

So the PA examined and took the history of a patient.

And the tribunal determined that the doctor failed in their duties for not examining the patient or taking a history themselves and in addition to the PA.

What exactly is the purpose of a PA in this medico-legal context?

Doctors should not be held to account for a PA making a mistake.

We didn't sign up to this.

Follow BMA guidance or say no to being a supervisor.

Otherwise it's YOUR license on the line.

Crystal clear statement from RCPEdin:

NO FURTHER EXPANSION of the PA workforce in the UK until these implications are addressed:

A) Patient Safety
- defining PA scope & ceiling of practice

B) Medical Training
- training of doctors should not be compromised for PAs

C)

GMC Hull York Medical School This paper makes some reasonable points but then trots out the same arguments effectively gaslighting doctors with concerns. I’m not happy with this very limited piece of a few interviews should be used to inform policy vs numerous royal colleges surveys pointing to the opposite.

UK public, you need to know this. Govt treats new doctors with utter contempt, and has no intention of giving you the number you actually need to look after you. It's just smoke and mirrors - meanwhile you're getting enormously shortchanged.

Neonatology jobs
Cardiology jobs

Jobs trained doctors would give anything for

Jobs requiring high levels of skill sets to keep the public safe

I repeat NHS England Workforce, Training and Education have lost control & it’s now a Wild West

With no Sheriff

This is not fair on the public

More evidence GMC were aware of PAs replacing Drs

In the linked report, the GMC were explicitly told that PAs were being used to fill gaps on Drs rotas & that costings should be done to compare employing PAs over Drs

What's being done to stop this?

What is a doctor? What can a doctor do, that no one else can?

The profession needs to have a long think about what the answers to these questions are as times change. Task by task, procedure by procedure, other professions are doing more and more.

What makes us unique?

🧵 I sincerely hope The BMA vigorously opposes this & that the Supreme Court rules AGAINST the parents. I see no purpose in naming the doctors in these cases. Where this has happened overseas, doctors have been attacked.

Donnchadha DB Which I’m not happy with.

Same for the concept of AA’s sitting the FRCA. I can respect their work and their professionalism whilst still accepting that to reach a certain point you need to have put in the ground work. Or we just let standard slip…

Plan by South Western Ambulance Service to introduce pre-hospital anaesthesia air-ambulance teams without a doctor on board. Is this appropriate? We want to start this discussion now.
