Time is nephron. Pt came to ER thursday pm, renal biopsy friday am, results friday evening=cast nephropathy; transferred to us sunday am, bone marrow biopsy performed and read sunday am,initiated VCd sunday afternoon.Never got dialysis. No plex.Renal recovery -beautiful (so far)

Example of treated chronic membranous nephropathy but persistent proteinuria. Spikes/luecencies on silver stain. Minimal IgG+ on IF. Mostly resorbed deposits on EM (stage 4/4) with very remodeled GBM -> ongoing proteinuria without 'active' deposits. #renalpath #nephrology

Low grade DNAJB-9 positive fibrillary GN hiding in a case of severe diabetic nephropathy. Clue was the smudgy IgG staining on IF. No definite fibrils in EM (probably due to segmental distribution). Probably mostly incidental finding in this case. #renalpath #pathtwitter
