When I first started as a GP trainee, it blew my mind how much time was needed for the work that wasn’t directly patient facing. Rarely ‘counted’ but crucial. Great to see this paper helping make this visible to support discussions about GP workload.

43% of UK medical academic workforce is 55+. 14% 66+. Fabulous to have these experienced colleagues but these figures raise concerns for me about how robust our academic pipeline is. (BMA membership stats)

More about the magic of continuity in healthcare - continuity in teams and familiarity is an enabler of effective teamwork and a barrier is transient team structures. Continuity - better for patients, better for their healthcare team! gmc-uk.org/about/what-we-…

It’s crucial generalists are vakued as much as our specialist colleagues. Pre 2000s we had more GPs than specialists, there is now 0.6 of a GP per specialist. Healthcare outcomes are better when the system has primary care and generalism at its foundations

Proud supervisor moment - PhD student Shalini Gupta presenting her work as a finalist at the ASME Research Paper of the Year awards. Her ethnographic exploration of the clinical learning environment is well worth a read Dr Mandy Moffat

What a depressing read. Medical students experiencing ‘non greeting behaviour’ on clinical placements. Not how we should treat our future colleagues or anyone. Say hello, make people feel welcome, learn their names. It shouldn’t need spelt out in a study.

The responsibility of all healthcare practitioners not to overdiagnose, overmedicalise or overtreat. A concept I wasn’t taught ‘back in the day’ but I hope we are getting better at now. For example, through training to apply Realistic Medicine in practice

Healthcare workforce planning is important and complex. Not just one in, one out. What are our current and future needs? Who is in the pipeline? How do we support retention? How do we work better across complex systems to do it better?

“Cognitive apprenticeship” is an absolutely brilliant term

The consultants I trained with who let me into their thought processes and let me know which bits are hard or stressful or upsetting for them were the best trainers

We spend a huge amount of time teaching our learners to prescribe. We should teach more about deprescribing - not just the why’s but also the how’s. Being confident to know what can and should be stopped and the skills to discuss that are critical.

Wow - not my favourite newspaper but great article- Scotland NHS highest managers please understand this message - the best way to keep A/E attendances down is to have enough GPs - we hold the risk others pass on

NHS colleagues stuck on endless NHS waiting lists for treatment. One may be made redundant as stuck on waiting list so long. Another a locum GP who can’t take on work as in pain and needs surgery. How does this make sense in a workforce crisis?

Sadly this graph doesn’t surprise me. Lots of different factors at play here e.g. changing trainee demographics from retiring GPs, a job that is less attractive than it was in the past. We need to take an evidence-informed approach to addressing this trend.

What a powerful image. Highlights how important it is as we evolve health professions education to think how we still enable our students and educators to feel embedded and belongingness in the medical and university community.
