We thank The Royal College of Surgeons of England for this clear statement, with safe surgical practice and trainees at its core

SCPs have a well defined scope of practice: performing parts of a lap cholecystectomy with or without supervision has never been acceptable

Following discussion yesterday at Royal College of Surgeons Council, here is a statement about Surgical Care Practitioner scope of practice

For me, the 2 most important things are patient safety & ensuring trainees get every opportunity

*walk into ring fenced day surgery ward*

Nurse shouts at me as I walk in : β€˜There’s no Gynae patients here’

It’s never gonna end is it?

This is really disturbing. A government minister, Maria Caulfield MP, spreading a blatant conspiracy fiction:
β€œPart of 15-minute cities is: if you go outside of your 15-minute area, you will have to pay a road tax.”
She doubles down on it when challenged.🧡bbc.co.uk/iplayer/episod…

🧡 showing just *some* of the things that surgical trainees going to the can sign up to practice on (for free), mostly in real tissue models.

Here is a trainee doing a vascular patch graft on a pig aorta that will then made pulsatile to check for leaks.

All surgeons have complications. How we manage them goes beyond the technical: acceptance, empathy, communication, self-support, navigating the medicolegal processes, wider support.

NUTA is a good way to help build situation awareness (SA) & actively reduce chance of error, whatever we are doing:

Notice what's going on around us
Understand it
Think Ahead...

E.g. what is this lorry going to do? Can I can safely switch lane to allow it to join the motorway?

I did 110 open cholecystectomies as an SHO including bile duct explorations- in 1990, as Reg, the consultant introducing lap cholecystectomies said is was the reserve of the SR - I did 6 as I felt I had a good foundation πŸ™. Just saying πŸ™πŸ™

Thoroughly impressed with the conciseness of this ASiT response to the SCP outcomes publication.

β€œMAPs wishing to expand their scope of practice have an established pathway that currently exists to allow them to gain the necessary qualifications: medical school.”

Prof Shafi Ahmed PhD FRCS Thank you for speaking up. I am absolutely horrified - my husband was in hospital for a month after this op - it is a very serious operation and shouldn't be underestimated.

Prof Shafi Ahmed PhD FRCS How much power do patients have? Can I insist on a surgeon? Would that be guaranteed if I asked? Would it be documented? Would they agree to my face and once I'm under let the associate do it? Remember the surgeon who burnt his initials on patients livers? Where were his team?

Prof Shafi Ahmed PhD FRCS What I wonder about , why consultants are allowing this ? Those patients are seen by and booked under the name of a consultant. Why they allow this care to their patients?

Prof Shafi Ahmed PhD FRCS Thank you Professor πŸ™πŸ½, this needs to be shouted from the roof tops loud and clear so everyone complicit in endangering pts lives in this way can hear & end these dangerous experiments using underqualified staff. We pts want ONLY trained *doctors* to deliver our healthcare.

Prof Shafi Ahmed PhD FRCS We need seniors like yourself to speak out against this! This should be brought up in meetings/conferences and rules put into place on what’s acceptable. Leaving it upto local trusts etc will be terrible for patients/trainees

Prof Shafi Ahmed PhD FRCS Thank you so much, Prof Ahmed, for speaking out loud and clear about the potential risks of this practice.

If only more of your colleagues would show similar leadership.
